If this is your first time visit, please be informed that you will be asked to fill out a few forms for your dental records. You may also print them from our web site, fill them out and then bring them to your next appointment. New patient forms.

As a courtesy, we call all of our patients as a reminder, 1 day before, to remind them of their appointments. If you are unavailable, we will leave a message asking you to call us back.

For your convenience, our office accepts different methods of payments including all major credit cards. A 10% discount for Senior Citizen over 60 will be subtracted from the total patient obligation.
